Admission
Your attending physician is the doctor responsible for your medical care and who arranges
for your admission to the Hospital. If you have any questions or concerns about your medical
care, be sure to discuss them with your attending physician.
Accommodation choices are 3-4 bed rooms, 2-bed rooms and 1-bed rooms. If you would like
accommodation in a semi-private (2-bed) or private room, check whether your medical insurance
covers the cost of this. The Almonte General Hospital will attempt to accommodate your request
for a semi-private or private room, although rooms are assigned on the basis of medical need.

Discharge
Members of your Health Care Team will discuss and assess the needs and challenges you will
face after being discharged from the Hospital and will help you develop strategies, including a
family conference if needed, to deal with them. This may include co-ordinating home
care/community supports and/or making physical adjustments in your home. The Hospital’s
Discharge Planner may also be asked to help you make informed decisions about your discharge.
When you have sufficiently regained your health or completed the required tests, your doctor
will attempt to notify you 24 hours before you are to be discharged from the Hospital.
The Hospital will provide you with any necessary prescriptions and an appointment for follow-up
care, if necessary. If you have any medications or valuables (in safe keeping at the hospital)
be sure to take them home with you.
We are committed to improving our services, and would welcome your feedback on your
experience at the Almonte General Hospital. Patient opinion questionnaires are available from
the nursing staff, should you wish to complete one.
On the day of your discharge from the Hospital, please arrange for transportation home prior
to 11 a.m. If necessary, a member of our staff will accompany you to the exit. We recommend
that a friend or relative accompany you home.
