Health Care Team
Your attending physician is the doctor responsible for your medical care and who
arranges for your admission to the Hospital. If you have any questions or concerns about
your medical care, be sure to discuss them with your attending physician.
In addition to your doctor, there is a team of professionals at the Almonte General
Hospital who work together, in collaboration with you, to provide your high-quality health care.
This Health Care Team includes nursing staff – both Registered Nurses and Registered
Practical Nurses – a pharmacist, physiotherapist, occupational therapist, mental health
nurse consultant, dietician, speech pathologist, volunteers and a discharge planner. Members of
the team, through individual contact with you and team conferences, ensure the ongoing
assessment, planning, provision, evaluation of your plan of care and treatment and discharge
back to your home or other facility.
Your input is important during all phases of your care, including your needs for discharge.
Please feel free to discuss any aspects of your care with members of the team who are working
with you. The Nurse Manager of your unit is responsible for making sure the unit runs as smoothly
as possible and is available to respond to your questions and concerns. Our commitment is to do all
we can to meet your expectations and goals.
