Rights and Responsibilities

The Almonte General Hospital is committed to providing care in a way that is humane and respects the dignity of all individuals. We believe patients have the right to make choice about their care and daily activities.

Certain treatments, especially surgical procedures, can only be done with your signed consent. Before signing any form, discuss the procedure with your doctor and make sure you fully understand what is being done and the expected outcome, including any risks involved.

It is also important to let us know if you have a substitute decision-maker, such as a Power of Attorney for Personal Care and/or Property.

During your stay at the Almonte General Hospital, always feel free to discuss your treatment and share any concerns you may have with members of your Health Care Team. They are responsible for your well-being and want to work with you to meet your goals and expectations. They will determine your needs and goals through individual discussion or in a meeting with you and/or your family members and Health Care Team members.

You are responsible for letting the Health Care Team know if anything is not going well. During your treatment, be sure to keep your doctor and nurses informed of your progress. If you don’t feel well, tell them. If you are in pain, tell them.

Good communication is an essential part of the treatment and caring process. For example, be sure to tell your doctor and nurses:

• If you have any hearing or vision loss, any speech/swallowing difficulties or other problem that may affect communication

• If you have any allergies to medications, latex, band-aids, or food

• If you are pregnant or think you may be pregnant

• Whether you would like to be called by your first name

• If you wish to be visited by a member of the clergy
