What to Bring
In-Patients
If you are being admitted to the Almonte General Hospital, please bring your Ontario Health Card,
Almonte General Hospital card (if you have one; if not, you will receive one when you are admitted),
medical insurance information relating to covering the costs associated with semi-private and private rooms and (if applicable)
your Workplace Safety and Insurance Board claim information (name and address of employer,
Social Insurance Number or claim number and the date of the original accident). Patients from outside Ontario or outside
Canada should bring their province’s or country’s insurance card.
Please bring any medications you are on with you to the Hospital and inform your physician and nursing
staff. These medications will be reviewed with you and kept until your discharge or returned to you to be sent home with a relative or friend.
Storage space is limited, so bring just a few belongings in a small bag. You will need two pairs of pyjamas
or two nightgowns (hospital gowns are provided if desired), a dressing gown, slippers with a sturdy tread, underwear,
a comb and/or brush, toothpaste, shaving supplies, shampoo, and, if applicable, eyeglasses (or contact lens supplies if applicable), hearing aid(s),
dentures and a denture cup.
Families are responsible for a patient’s personal laundry.
Please leave money, jewellery and credit cards at home. If you do bring valuables, they may be placed in our
business office for safekeeping when you are admitted. The Hospital is not responsible for lost or stolen items.
Cell phones, personal televisions, laptop computers and appliances such as hairdryers, coffee makers,
fans and humidifiers are prohibited. Telephones can be rented at the Hospital for $3/day (taxes included). Televisions can be rented at the
Hospital for $4/day or $25/week (minimum two days’ rental required).
Out-Patients
If you are coming to the Almonte General Hospital as an out-patient, please bring your Ontario Health Card, medical insurance information and
(if applicable) your Workplace Safety and Insurance Board claim information (name and address of employer, Social Insurance Number or claim number and the date of the original accident).
If you are having a test, be sure to bring the completed requisition from your doctor.
